- Falls management and prevention referral checklist
Queensland Health has developed a best practice referral checklist to facilitate better management of older people who have had falls while living in the community. The checklist assists health professionals to refer older people to suitable health care providers from different disciplines and agencies. The checklist is consistent with Queensland Health's Falls Prevention Best Practice Guidelines for Public Hospitals and State Government Residential Aged Care Facilities (2003), and was developed by the Brisbane South Centre for Health Service Integration as part of the Falls Management and Prevention Project.
