Fall continuum - Stay On Your Feet
To reflect the movement of an older person across the three different settings of community, hospital and residential aged care, Queensland Stay On Your Feet developed a model for falls prevention across the health continuum. Where the person is located along the continuum depends on their health status and falls risk. The model lists how health professionals can be involved at each stage and the best evidence based interventions for each older person’s situation. The model seeks to deliver a coordinated approach to the delivery of falls prevention programs between all sectors from the wider community, primary health care, acute, residential aged care facilities and rehabilitation sectors.
