Review of mental health services following Moorooka incident
The Director-General of Queensland Health ordered an independent investigation into the mental health treatment provided to former patient, Anthony O’Donohue, following a tragic incident at Moorooka on 28 October 2016.
The investigation report, led by forensic psychiatrist Professor Paul Mullen, was completed in April 2017.
As a result of its findings, Metro South Hospital and Health Service commissioned their own investigation into the prevailing culture, practices and protocols within their mental health services.

Both reports contain a series of recommendations aimed at addressing concerns raised during the investigations and, ultimately, improving the reliability and safety of public mental health services for consumers, carers and the broader community.
