  • Julian’s Key Health Passport is a free communication tool for people of all ages. It helps you communicate your health needs and receive the best care.

    Available in both printed and digital formats. Your Health Passport includes essential information about you, including how you communicate and your support and care needs.

    You control how much information is filled out, and you decide when and who can see it.

    Fill it out on your own, with your carer or by calling 13 HEALTH on (13 43 25 84).

    Always bring your Health Passport when visiting a hospital or community health clinic. Healthcare staff will use it to provide you with the best care.

    If your care needs change, it's important to update the information in your Health Passport.

    Your Health Passport doesn’t replace regular treatment and care planning.

  • How to get your Julian's Key Health Passport.

    Julian’s Key Health Passport is available in both printed and digital formats.

    To get your Health Passport, open your internet browser.

    In the search bar, type ‘Julian’s Key Health Passport’ and press enter.

    Locate and click on the official website link.

    Once the website opens, scroll down to view the available versions.

    You can choose between a printed or digital version.

    If you'd prefer a printed version, email JuliansKey@health.qld.gov.au or call 13 HEALTH on 13 43 25 84.

    A physical copy will be mailed to you for free, and may take a few days to arrive.

    If you'd prefer a digital version, click the 'Download' button.

    It will be ready to use right away. You can fill it out digitally, or print it to complete by hand.

  • How to complete your Julian's Key Health Passport.

    Once you have your Health Passport, fill out as much information as you like.

    Remember, you don’t have to complete it all at once.

    If you need someone to help you fill it out, you may ask a family member, friend or carer.

    You can also call 13 HEALTH on 13 43 25 84.

    Save any updates or changes you make and store it in a safe place.

    Let your carer know it’s important to bring to regular treatment and care planning appointments.

  • How to print your Julian's Key Health Passport.

    Open the PDF by double clicking on it.

    Select 'File' and then select 'Print'.

    Under 'Pages to Print', select 'Pages', and then type in the page numbers you want to print (for example: 1–10).

    Finally, select 'Print'.

    Now wait for the printer to finish printing your Health Passport.

  • How to use your Julian's Key Health Passport.

    Always bring your Health Passport when visiting a hospital or community health clinic.

    Show your Health Passport to the healthcare staff before they treat you.

    If you are unwell or unable to communicate, your carer can show your Health Passport for you.

    With your consent, healthcare staff will read your Health Passport.

    They may copy information about your care  and treatment into your medical record.

    If your care needs change, it’s important to update the information in your Health Passport.

    Remember to ask for your Health Passport back once the healthcare staff have all the necessary information. It is yours to take home.
