Care Coordination Service Centre
The Care Coordination Service Centre (CCSC), is a joint effort between the Cairns and Hinterland Hospital and Health Service, Torres and Cape Hospital and Health Service, and Queensland Aboriginal and Islander Health Council.
Our services
Our centre is in Cairns. Our services organise health care for our patients. We offer wide-ranging support for priority patients referred for planned, specialist care in an outpatient setting.
Our focus is on making care close to home and easing the patient journey.
We support:
- appointments—we can assist with specialist appointment coordination, including virtual and face-to-face. We ensure patients receive the care they need, when they need it.
- logistics—we can assist with travel and arrangements for somewhere to stay, aiming for a stress-free experience for our patients.
- system navigation—we know the healthcare system can be complex. Our services help patients find the care they need.
- cross-provider liaison—we connect our patients to different health services to ensure seamless and wide-ranging care.
Patient Experience Officers
Cultural context in healthcare is vital. We use inclusive practices that respect and embrace the unique needs of our Aboriginal and Torres Strait Islander patients.
Our Patient Experience Officers support patients in culturally appropriate ways. This support helps Aboriginal and Torres Strait Islander people and their families during their Cairns visits for planned care.
Priority Patient Dashboard (PPD)
To improve the patient journey, we've built the Priority Patient Dashboard. This opt-in software platform presents a unified look at patient data for specialist outpatient appointments.
Service providers using the PPD can:
- manage priority patients needing care co-ordination
- discover any service gaps
- see where patients are on their care pathway.

The launch of the Service Centre and Priority Patient Dashboard is planned for mid-late July 2023.
