Submitting samples

Where possible, please complete these forms electronically. Print and send the form with your submission.

To send samples for testing to the Public Health Laboratory you will need a completed sample submission form (XLS, 73.5 KB).

Hazardous materials

Strychnine and other hazardous materials for destruction should be sent to Cleanaway. For more information and advice please contact tesqldsales@cleanaway.com.au.

Dimethoate dip

Dimethoate dip used in fruit fly chemical treatment needs to be accompanied by the 'Dips' sample analysis form (DOCX 48 kB). Each sample being submitted should have its own form.

Legal samples

Legal or potentially legal samples must also be accompanied by a Chain of Custody form. Do not send forensic samples to Public Health Property Point.
