MASS Client Reference Cards

The Medical Aids Subsidy Scheme (MASS) introduced MASS Reference (ID) Cards in March 2022 to assist consumers when applying for MASS services or when contacting MASS.

MASS is able to send this card with your MASS reference number (“B Number”) upon request for future reference to assist you with new or re-application and to provide information for your local health professional, equipment provider or contacting MASS.

How do I get a MASS reference card?

If you have not applied to MASS before, you will need to apply for MASS services.

Contact MASS and request a MASS reference card after the approval of your first application.

MASS clients can also request a new or replacement card completing the below form or by emailing MASS-Admin@health.qld.gov.au

The Medical Aids Subsidy Scheme (MASS) is collecting your personal information on this form to confirm your identity and send you a MASS reference card to your nominated postal address and/or to update your details. Your personal information will be used by authorised officers within the Medical Aids Subsidy Scheme.
