HealthPathways
CPCs are located on the Queensland Health website and are embedded in HealthPathways.
Many clinicians will first come into contact with CPC as part of making a referral using HealthPathways.
HealthPathways is a web-based portal with evidence-based information on the assessment and management of common clinical conditions including referral guidance. General Practitioners write the pathways with support from local GPs, hospital-based specialists and other subject matter experts.
Clinical Excellence Queensland is working with Hospital and Health Services and Primary Health Networks across Queensland to implement HealthPathways. Pathways are being added regularly across all local sites.
The CPC program team continues to work closely with the HealthPathways Coordinators across the state and with other services to ensure that the pathways contain the most up-to-date information.
