There are 3 ways to access patient resources for advance care planning:

Order from WINC

To order, you must have a WINC account.

The cost includes delivery to anywhere in Queensland. Orders should arrive within 1 business day.

1. Log into your WINC account
2. Search for the resources using the item codes below:
   • Statement of Choices (SoC) forms, including tip sheets (50 pack) - code: SW1174 or SW1175
   • ACP folder and resources, including postcard, brochure, Statement of Choices (form A and B) and OPG information sheets (25 pack) - code: SW1219
   • ACP brochures (50 pack) - code: SW1186
   • Advance care yarning brochures for First Nations people (50 pack) - code: SW1262

We have limited supplies. We can provide up to 50 advance care planning brochures and 50 Statement of Choices forms per request.
