ACP is recognised as a cornerstone of person-centred quality care for older people, particularly those living in residential aged care facilities (RACFs).

To provide quality end-of-life care in line with accreditation standards, each RACF should appoint staff to be responsible to embed ACP as part of routine clinical care.
