Accessing ACP documents

Completed advance care planning (ACP) documents that meet the ACP checklist criteria, are uploaded to a person's Queensland Health electronic hospital record within the ACP Tracker, which supports:

  • individuals and substitute decision-makers to know relevant ACP documents can be easily obtained by clinicians when needed
  • clinicians to have direct access to a person’s ACP document(s) and to have confidence in the quality of the information contained within it.

ACP documents may be provided to Queensland Health by individuals, hospitals, health services, GPs and residential aged care facilities across Queensland. The ACP documents we receive include:

  • Advance Health Directive (AHD)
  • Enduring Power of Attorney (EPOA)
  • Queensland Civil and Administrative Tribunal (QCAT) Decisions
  • Revocation documents
  • Statement of Choices (SoC)

The ACP Tracker

The ACP Tracker (available via The Viewer or in ieMR from the Advance Care Planning menu item) enables clinicians to have direct access to uploaded ACP documents and comments to support the provision of quality end-of-life care across service settings.

Making the documents available in The ACP Tracker means that clinicians can access them when and where they are needed.

The Viewer is a Queensland Health web-based application that collates comprehensive data from multiple Queensland Health systems. It provides real-time vital clinical information regardless of where the health professional or patient is located within the state.

The ACP Tracker:

  • supports tracking of advance care planning interactions (discussion, completion, review and declined)
  • provides real-time access to current documentation
  • is available to authorised users across all Queensland care settings including:
    • public and private hospitals
    • residential aged care facilities
    • general practices
    • Queensland Ambulance Service
    • community health services.

Don't work for Queensland Health?

Eligible health professionals outside of Queensland Health can apply for secure, read-only access to their patient’s Queensland Health records and the ACP Tracker via the Health Provider Portal (HPP).

This read-only, online access allows authorised health professionals to view public hospital information including ACP documents, acute resuscitation plans, appointment records, radiology and pathology reports, treatment and discharge summaries, demographics and medication details.
