Queensland Health Clinical Standard for Cardiac Rehabilitation Services

The objective of the standard (PDF 454 kB) for CR Services is to ensure that standardised and evidence-based content informs the delivery of CR in Queensland. Specifically: to enable service delivery that is evidence-based, person-centred, culturally aware, flexible, applicable to all modes of delivery and provides consistent and clear messaging.

Queensland Health Cardiac Rehabilitation Services: Model of Care document

This document  (PDF 1288 kB) describes the patient journey through the models of care that are commonly delivered from Queensland CR programs and how this is captured within QCOR. It should be read in conjunction with the Clinical Standard, QCOR user guide and the CSANZ position statement of exercise (also on this page).

This position statement (PDF 784 kB) provides pragmatic, evidence-based guidance for the assessment and prescription of exercise and physical activity for cardiac rehabilitation.

Queensland Health Cardiac and Pulmonary telerehabilitation guideline

This is a practical guide (PDF 348 kB) to help clinicians (particularly exercise therapists) implement virtual exercise and education programs for those with cardiac conditions (including heart failure) and chronic pulmonary disease.

Additional documentation exists to support the establishment of this Pulmonary and Cardiac Telerehabilitation (PaCT) program based on learnings of establishing and implementing PaCT across the Far Northern, Central Western and Western corridors.
