The Community Palliative Care Clinic Box contains tailored resources for families, health professionals and clinical services to support the provision of quality end-of-life care for people who choose to be cared for at home.
Note: it is very important to note that the clinical processes of carers managing subcutaneous medicines at home have not changed as they continue to be consensus-based best practice. Existing resources in individual caring@home packages for carers can still be used.
This free package provides practical information that carers can refer to at any time and is appropriate for all jurisdictions in Australia.
PallConsult, funded by Queensland Health, has partnered with caring@home to produce a limited amount of Community Palliative Care Boxes to support the provision of best-practice palliative care in Queensland.
Queensland services providing community palliative care can order 1 box per site. The Resources Box is designed to be stored centrally and contains resources that can be used with several patients and families. Printed resources contained in the Resources Box can also be downloaded and printed to restock the Box.
