Requesting surveillance data
Public Health Intelligence Branch, within the Queensland Public Health and Scientific Services division, is the main point of contact for customers making enquiries in relation to access to data held in applications managed by the branch.
Public Health Portfolio Office facilitates and provides governance through the Data Access Request Form process.
Customers requesting access to data are required to complete the Data Access Request Form. Completion of this form is part of the governance and decision-making process for data access requests. Information contained within the Data Access Request Form assists in determining if the request is compliant with relevant requirements.
Final authorisation is determined by the Data Custodian(s) after all relevant requirements have been satisfied.
Notifiable conditions
Disease surveillance reports
Disease prevention and control information, together with disease surveillance reports, are regularly updated and made available in the public domain.
Individuals or organisations requiring historical and/or more detailed data will usually need to apply for permission to obtain them.
Weekly notifiable conditions reports
Monitored medicines data
Monitored medicine-related information collected under the Medicines and Poisons Act 2019. This includes prescribing and dispensing records, details of regulatory approvals and Queensland Opioid Treatment Program admission/discharge data.
How to submit a request to access data held in Public Health Intelligence applications:
- Complete the Public Health Intelligence Branch Data request form (PDF 350 kB)
- Once completed, submit the Data Access Request Form to Public Health Intelligence Branch via email: EPI@health.qld.gov.au
The Data Access Request Form will only be accepted when all sections of the form have been completed and required approvals from the application delegates have been obtained.

Access to identifiable information for research
If access to identifiable or potentially reidentifiable data for research is being requested, an application form under Chapter 6, Part 4 of the Public Health Act 2005 (PHA) is required.
