    1.1 This web-form or other online platform (Form) has been prepared by the State of Queensland acting through Queensland Health (ABN 66 329 169 412) and/or its contracted service provider(s) (Queensland Health, we or our).  We maintain this Form to collect information from you in connection with the Queensland Community Pharmacy Pilots (the Pilots) as well as to provide you and the public with information in respect of the Pilots.  This document sets out the terms and conditions that govern the use of this Form (Terms).  Please review these Terms carefully, especially the disclaimer and limitation of liability sections. By accessing this Form (other than to read these Terms for the first time) and continuing to use this Form, you accept and agree to these Terms.  You must not use this Form if you do not agree with these Terms.

    2. Use of this Form

    2.1 You are responsible for all of your activity in connection with accessing and using the Form. You must not use this Form for any activities that breach any laws, infringe any party’s rights or breach any content requirements or codes promulgated by any relevant authority. You must not distribute through this Form any inappropriate communication or any virus or other disabling code in any form.

    2.2 You must not bypass any security mechanisms imposed by this Form, or tamper with, hinder the operation of or make unauthorised access or modifications to this Form.

    6.4 We may utilise “cookies”, Google Analytics and other tools on our website in accordance with our Website Privacy Statement.

    7. General

    7.1 If any provision of these Terms is found to be void, unlawful or unenforceable for any reason, then that portion of the provision which creates the invalidity, unlawfulness or unenforceability will be severable from these Terms and will not otherwise affect the validity and enforceability of any remaining provisions.

    7.2 No waiver by us of our rights under these Terms shall be deemed a waiver of any other term or provision and shall be limited to a single waiver limited to the specific circumstances under which such waiver was granted.

    7.3 To the extent permitted by law, we may change these Terms at any time.  Please check these Terms at regular intervals for any changes.

    7.4 The law of Queensland governs these Terms. The parties submit to the non-exclusive jurisdiction of the courts of Queensland and the Commonwealth.
