RACF resident assessment tools
Use of validated assessment tools to evaluate residents with acute care needs supports improved health outcomes. The assessment tools recommended here are based on:
- Published literature in both residential and acute care settings evaluating feasibility, clinical validity and inter-rater reliability
- Ability to integrate assessments with acute care systems, allowing for evaluation of temporal trends in response to treatments / care
The below tools are referred to in individual pathways – all users should ensure that they undertake training in their use and understand how to reliably perform these assessments.
It is acknowledged that individual providers may have alternate tools that are used.
