| Home Care Services: Access Brochure
The Queensland Health Home Care Service is funded by the Home and Community Care (HACC) Program to provide basic support services that assist eligible clients to live independently in their own home. Queensland Health services such as Home Care are designed to be flexible and respond to your needs. The type and level of services that can be provided depend on the client's screened and assessed need. The main service type provided is domestic assistance. However, the Home Care Service may also be funded to provide Personal Care, Social Support and In-home Respite services. Home Care Services assist people to maintain their independence in their own home. This brochure explains what the Queensland Home Care Services is, what services that are offered, eligibility to access the services and Home Care screening and assessment process. |

Home Care Services: Service Charter The Queensland Health Home Care Service Charter sets out your rights and responsibilities about the Services we provide to you. The purpose of this document is to formalise agreed Service arrangements between Home Care Services and each individual client.
